Service Access Supervisor

Responsibilities:

• Supervise the work of three direct reports in the Service Access team
• Streamline client flow from intake to service delivery, assess potential clients for service eligibility and assign assessment schedules using the database system
• Support the team in developing and monitoring Care Plans offering additional coaching and support with complex care situations
• Manage, in rotation, the support for on-call phone during non-traditional hours
• Participate in continuous quality improvement processes, assist with research and development of best practices and promote high standards of client-focused customer service
Qualifications/ Skills:
Education: Bachelor of Social Work or an equivalent degree and experience
Experience: Minimum of 2 years supervisory experience with community service sector experience, an asset
Driver’s license and access to a vehicle is an asset
Other: Experience leading quality improvement initiatives
Proficient in using MS Office suite; GoldCare an asset
Strong interpersonal and customer service skills
Ability to communicate effectively in English (verbal and written); other languages an asset
A self-starter, able to work independently as well as in a team
Knowledge of Ontario Health Teams, an asset
Current CPR & Standard First Aid Certification
Current Vulnerable Sector Screening Check
